Aquatic Safety Research Group is committed to tracking verifiable incidents of Shallow Water Blackout. Your help is needed in documenting incidents of SWB.  Facility and Contact information will be kept confidential but is required for verification and data collection purposes. Please email us your SWB incident, and include the following information:

• Your title
• Company
• Street Address
• City, State, Zip
• Country (if not US)
• Telephone / Fax
• Date of Incident
• Facility type:
  • Residential Pool
  • Competitive Pool (Specify indoor or outdoor)
  • Leisure Pool (specify indoor or outdoor)
  • Freshwater beach

• Gender of victim (Male / Female)

• Activity prior to the incident:
  • Breath holding, stationary
  • Underwater distance competition
  • Lap swimming
